Inc.”) Your Insurance Center operates the portal required to collect
information provided by and pertaining to you. As such, this notice describes
the privacy practices of Your Insurance Center, GAIN, Inc., and their
respective employees, staff, independent contractors and other personnel
(collectively referred to herein as “Your Insurance Center”).
All of these individuals follow the terms of this notice.
This website is designed to serve the Subscribers of Your Insurance Center
and the participating providers of those services ("Providers").
This Web Privacy Policy attempts to distinguish between Subscribers and Providers.
If it is unclear to which group a provision applies, or if a provision is not
specifically limited to Subscribers or Providers, then the provision applies
to both Subscribers and Providers.
Your Insurance Center wants you to be able to protect your privacy on the Internet.
1. INFORMATION COLLECTED BY YOUR INSURANCE CENTER
Get a Quote Service: When
Subscribers utilize our Get a Quote service, we collect non-personally
identifiable information about our Subscribers, including zip code.
Provider Registration: When a Provider registers to use this
website, we collect information about that Provider over the website, including
their doctor number, password and/or location number.
Provider's Request of Member Eligibility: Within
the Provider area of our website, Providers can send us requests for member
eligibility, which lets a Provider know whether or not a certain Subscriber
is eligible for Your Insurance Center services. As part of this process,
we collect the plan number, Subscriber name and individually identifiable
number assigned to each Subscriber (i.e. social security number) in order
to process the Provider's request for eligibility.
Aggregate Information: Your Insurance Center collects
aggregate, non-personal information about its Subscribers and their usage
of this website and any related services. The web server supporting this
website collects internet protocol ("IP") addresses and/or domain
names, but not the e-mail addresses, of visitors. This information is not
used to identify any individual Subscriber.
Cookies: Your Insurance Center also collects information
about your use of the website through the use of cookies. Cookies are small
pieces of information passed by the web server to your browser. We use cookies
to identify you to the web server during page requests. We also use cookies
to tailor your experience on our website. You can control the use of cookies

Information from Children: Your Insurance Center

- Insurance Providers: Personally identifiable information
is collected about our Subscribers during the eligibility submission
process. This information will only be used for legitimate business
purposes. Specifically, we use this information to process and determine
the Subscriber’s eligibility for the particular insurance product
being quoted. Personal and medical information collected is to be used
specifically for application submission, underwriting and billing purposes.
Please be assured that the insurance providers are also obligated to
maintain the confidentiality of this information.
- Affiliated Organizations and Contracted Entities: Your
Insurance Center shares your personal information with its and the
insurer’s affiliated organizations and contracted entities involved
in processing your insurance application, servicing your insurance
application, coordinating information with the insurer or contacting
you. Please be assured that the affiliated organizations and contracted
entities may only use your personally identifiable information in providing
these services and are contractually obligated to maintain the confidentiality
of this information.
- Service Providers: Your Insurance Center shares
your personal information with service providers for enrollment verification,
marketing purposes, underwriting functions and the like. Please be
assured that the service providers are also obligated to maintain the
confidentiality of this information.
- Plans: Your Insurance Center shares your personal
information with Subscribers' health plan, employer or plan sponsor
("plan") in order to perform Your Insurance Center 's contractual
obligations with these plans to provide accurate and complete insurance
applications. Please be assured that these plans are also obligated
to maintain the confidentiality of this information.
- Required by Law: In cases, where required by law or the court, Your Insurance Center may be forced to release information regarding our Subscribers or Providers to permit the investigation of credit card fraud, illegal use of the Internet or some other cases of misconduct on our website.
Subscriber Contact: Your Insurance Center
may use your personally identifiable information to contact you about
the application process. For example, you might be contacted for information
needed to complete or process your application or to inform you of the
status of your application. Your Insurance Center might also contact
you to survey your experience with our service or to inform you of additional
products and services. If you do not desire to continue receiving such

Aggregate Information: Your Insurance Center

information about our Subscribers or Providers.
Cookies: Information collected via cookies
helps Your Insurance Center determine what areas of our website are important
to our Subscribers and Providers. Your Insurance Center does not use
cookies to identify individual Subscribers or Providers.
Change of Ownership or Control: In the event that
Your Insurance Center becomes owned or controlled by other individuals or
entities, the information we have collected may be transferred to such party
or parties. In this case, such individuals or entities will follow this Web
Privacy Policy in the place of Your Insurance Center's current owners.
3. HOW YOUR INSURANCE CENTER CONTROLS SUBSCRIBER INFORMATION

Center uses all reasonable efforts to keep information from our Subscribers

Passwords: As part of the registration process,
